COVID-19 Patient Assessment Form
Dear Valued Patients and Families,
At this time, we would like to relay some information regarding our practices during this time in our office.
We are taking the COVID-19 situation very seriously in order keep our clients, their families and our staff healthy. We have daily cleaning staff and are being diligent with consistently sanitizing and disinfecting surfaces using hospital grade products on all surfaces between patients.
We would appreciate that if you have traveled out of the country or across the border, been in contact with anyone infected by COVID-19, OR are showing ANY cold or flu like symptoms to avoid coming in to minimize the spread and be respectful of other patients and office staff.
If you are unable to come into your appointment, please notify the office as soon as possible. We would appreciate if you are able to give the 24 hour notice as per office policy however, if you do need to cancel a same day appointment the cancellation fee will not be charged due to current circumstances. Those who have booked an appointment with us will receive a link for a form to fill out before coming to the office. For your convenience, a link to the form is also below.
